TITLE III-B Services

Find services in your community
  • Assisted Transportation - Providing transportation and an escort to older persons who have difficulty using regular transportation. While regular III-B Transportation usually picks up a person at the curb, Assisted Transportation is "door-to-door", and the escort will often wait with the older person, at the doctor's office or other destination.

  • Case Management - Assisting older persons who are experiencing diminished functioning capacities, personal conditions or other characteristics which require the provision of services by formal Grantees with access to those needed services and the coordination of their care.

  • Individual Needs Assessment - A face-to-face contact with a potential or existing recipient of supportive and/or nutrition services to examine medical, social and psychological factors relating to the need for service. This determination will include analyzing, evaluating and verifying current, full and complete information obtained at this personal assessment, in addition to any information from collateral sources if necessary.

  • Information & Assistance - A service for older individuals that provides current information on opportunities and services available within their communities; assesses the problems and capacities of the individuals; links the individuals to the opportunities and services that are available; and, to the maximum extent practicable, ensures that the individuals receive the services needed by the individuals by establishing adequate follow-up procedures.

  • Legal Assistance - Arranging for and providing assistance in resolving civil legal matters and the protection of legal rights, including legal advice, research and education concerning legal rights and representation by an attorney at law, a trained paralegal professional (supervised by an attorney), and/or a law student (supervised by an attorney) for an older person (or his/her representative).

  • Ombudsman - (funded with Title III-B, Title VII and State General Revenue funds) Identifying, investigating and resolving complaints made by, or on behalf of, residents of long-term care facilities and providing services to assists residents in protecting their health, safety, welfare and rights.

  • Residential Repair & Renovation - Assisting older persons with physical or cognitive problems to maintain or adapt their homes to meet their specific needs.

  • Transportation - Transporting older persons to and from community facilities and resources for the purpose of acquiring and receiving services, participating in activities and attending events in order to reduce isolation and promote successful independent living.

 

TITLE III-C Services

  • Nutrition Services - Providing nutritious meals in congregate meal sites and delivering meals to older persons who are homebound because of illness or disability. Find services in your community

     

TITLE III-D Services

  • Medication Management, Screening & Education Services - Education and assistance to older persons to use medication properly, manage health problems, and prevent incorrect medication use and adverse interactions.

TITLE III-E Services

Services authorized under Title III-E of the Older Americans Act are also known collectively as the National Family Caregivers Support Program. These services are similar to services authorized under Title III-B that bear the same or similar names with the exception that the Title III-E services are targeted to Family Caregivers and GRG (grandparents raising grandchildren). The Agency is required to designate community-based Caregiver Resource Centers for the collocation and coordination of services for older persons and their Caregivers.   • Case Management - Case Management for Family Caregivers and GRG. Case Management services under the National Family Caregiver Support Program determine eligibility for Title III-E respite services.

  • Counseling - Counseling service for Caregivers.

  • Respite Services - Providing temporary, substitute support to allow Family Caregivers a brief period for rest or to attend to other needs.

  • Legal Assistance - Legal services, education and advocacy for Family Caregivers and GRG.

  • Education - Annual Conference for GRG.
